
General: The patient is awake, alert, and oriented times three. The patient appears to be in no acute distress.
Head: Normocephalic, atraumatic, hair and scalp normal
Face: Patient has a ___cm laceration located ___. No palpation of facial fractures. No hematomas.
Eyes: Pupils are round and reactive to light bilaterally. Extraocular muscles are intact. No nystagmus is appreciated. No chemosis. The sclera are clear without injection. No proptosis or hyphema was noted.
Ears: Inspection of the auricles is unremarkable bilaterally. Tympanic membrane is round and intact without effusion, well-aerated, with good light reflex bilaterally. Canals are clear. Mastoid area is non-tender, non-erythematous, and nonfluctuant bilaterally. No foreign bodies. No hemotympanum. No CSF otorrhea. No tympanic membrane perforation. No battle sign. Palpation reveals no auricular hematoma.
Nose: The nares are patent. No polyps, septal deviation, mucopurulence, epistaxis, septal hematoma. No septal dislocations, no mucosal tears, no external deformities, no ecchymosis, no open fractures. No palpable crepitus, tenderness, step offs or mobility. No signs of CSF leak.
Neck: Supple. Trachea is midline. No palpable cervical lymphadenopathy. No palpable masses bilaterally. Normal carotid pulses bilaterally with out bruit. Full extension and range of motion of the patient's neck. Thyroid non-tender without nodules. No lacerations. No hematomas. No ecchymosis. No crepitus or tenderness.
Mandible and Oral Cavity:
Floor of mouth negative for hematoma. Normal dentition. No missing teeth or dental fractures. No palatal fracture. No intraoral or lip lacerations. Stenson's duct is intact and not involved by the laceration. No palpable mandible fractures. No hypoesthesia of the lower lip and chin. Normal occlusion.
Normal healthy dentition. Tongue is midline and mobile. Oral mucosa/buccal mucosa is pink and moist. ___+ tonsils are nonerythematous without exudates. Uvula is midline. No asymmetry to the natural arch of the palate. Floor of mouth shows no mass upon palpation. Tongue is without mass as well.
Neuro:Cranial nerves II through XII are grossly intact. Voice is strong.
Pulmonary:Patient has non-labored breathing. No stridor is audible.
